DOT requirements
Existing Armadillo Patient Cost: $85.00 (includes basic urinalysis)
Cost if UDS required: $150.00
Basic Requirements:
1. At least 20/40 correctable vision in each eye as well as both combined
2. Ability to distinguish colors found on traffic signals
3. Forced whisper test to detect adequate hearing
4. Controlled blood pressure below 140/90
5. Maximum blood glucose of 200
6. No use of controlled substances including narcotics, amphetamines, and/or habit-forming drugs
Before arriving for your appointment, please do the following:
1. Arrive 20 minutes prior to your appointment
2. Bring your completed DOT form along with your driver’s license and pocket certificate
3. Bring corrective lenses if routinely used for vision
4. Bring all prescription medications and any over the counter medications
5. Bring a list of any specialists you are currently seeing including their phone numbers
6. Take all routine medications prior to your appointment including blood pressure medications
7. Be prepared to leave a urine sample
Diabetics: Please bring a list of all medications and the name of the prescribing physician. Copies of your most recent blood glucose results and an A1C result completed within the last three months are necessary.
Any individual with a heart condition: Please bring a copy of your most recent stress test and/or a release letter from your cardiologist. If you have had a CABG and/or stent placement, you must have written clearance from your cardiologist stating you are safe to drive.
Sleep Apnea: Bring a copy of the your latest sleep study results along with a copy of your CPAP compliancy notes
